Form 1095-C: Employer-Provided Health InsuranceThe College processed 1095-C forms to all employees who were made an offer of health insurance coverage and/or had health insurance in 2022. Forms are available in Workday and were only mailed to those who elected a paper copy.
Individuals can now self-attest on their tax returns that they had health insurance coverage. Some tax preparers may want to see the form showing coverage for part or all of 2022. You may also receive multiple 1095 forms if you have multiple employers, your spouse works, or you are eligible for some government programs.
If you are covered by Kaiser Permanente through Montgomery College, visit the Kaiser Permanente website to print or download a copy of your form, or call the Member Services number on the back of your Kaiser ID card. Kaiser 1095-Cs are not in Workday.

Finding Relief from Post-Traumatic Stress Disorder People who have been through or seen traumatic events, like war veterans or people who have been abused, often have a hard time getting rid of disturbing memories and anxious feelings. Post-traumatic stress disorder (PTSD) is a condition that you may have if you've been through a traumatic event that is still bothering you and making it hard for you to do things and enjoy life. This disorder can be successfully treated. Do not be afraid to seek support.
PTSD is a mental condition that can happen after a scary event, like a scary combat mission, an earthquake, a car accident, being kidnapped, being raped, or coming close to dying. The tragedy is ingrained in the person's memory, whether they were directly impacted by it, witnessed another person suffer as a result of it, or witnessed a mass disaster. Most of the time, the person has also been hurt physically, but psychological scars are often the hardest to get rid of.
Don't let PTSD control you. Learn to control it by seeking help and educating yourself on the topic. Get involved in a support group. Ask friends and family to help you get through this difficult period. When you feel anxiety emerging, attempt to rationalize your fears and relax. Try breathing exercises, imagery techniques, and meditation to calm down.

Required Training: “Preventing Sexual Harrassment and Workplace Discrimination and Retaliation”
In this online course, participants will be reminded of their role in maintaining a culture of integrity and respect in the workplace. Participants will learn to define abusive conduct, harassment prevention, reporting, bullying, and abusive conduct.
This class has a minimum time requirement of 55 minutes to remain in compliance with state laws. Therefore, you will not get credit for the course if you complete it in less than 55 minutes and will be required to retake it for credit.
Make sure you schedule enough time to complete the course and periodically save your progress. To save your progress, ensure you use the internal course ‘X’ found at the top right-hand corner of the course. This will bookmark your progress and you will return to where you stopped. Also, saving your progress prevents the loss of your progress should you inadvertently sign out before the end of the training, experience a power surge or outage, or the course idle and time out. View additional tips in the March 1, 2023 issue of Employee Matters.

We will gladly return your call within 24–48 business hours. We ask that you refrain from making back-to-back calls without leaving a message. The HR Help Desk staff may be assisting other employees onsite or already assisting someone by phone when calls come through.
If there is a specific HRSTM team member you need to meet with in person, please make an appointment during these hours. Employees may visit the HRSTM suite to use a computer if they need to make changes in Workday that must be completed at an MC campus or building location for security purposes, such as Payment Election changes.
All employment verifications without full Social Security Numbers (SSN) should be emailed to the HRSTM inbox, which includes Public Student Loan Forgiveness forms. Make sure your SSN is not on the form when you send it to us; add that yourself before you mail it to the Department of Education. If your form already has an SSN and you cannot remove it, please make sure you password protect the file before you email it and send the password separately. Any employment verifications that require department-specific information should be completed by a supervisor before being forwarded to HRSTM for service date and/or salary verification.
